What is Cirrhotic Cardiomyopathy?

Did you know?
Advanced liver disease doesn’t just affect the liver — it can silently damage the heart too.

“Cirrhotic Cardiomyopathy” is a lesser-known but serious heart complication seen in patients with chronic liver cirrhosis.

It means the heart becomes weak, stiff, and fails to pump blood properly — especially during stress or illness.

Why Does This Happen?

Liver cirrhosis leads to:

  • Hormonal imbalance

  • Fluid overload

  • Electrolyte disturbances

  • Inflammation & Stress on the heart muscle

This gradually affects heart function, even if there is no prior history of heart disease.

Symptoms of Cirrhotic Cardiomyopathy

Often Silent — but worsens during stress (infections, bleeding, surgery, liver transplant)

Watch for:
✔️ Easy Fatigue
✔️ Shortness of Breath on Walking
✔️ Swelling in Feet (Edema)
✔️ Palpitations (Fast Heartbeat)
✔️ Low Blood Pressure
✔️ Chest Discomfort
✔️ Dizziness or Blackouts

Why is Early Detection Important?

In cirrhosis patients, a weak heart may:

  • Increase risk during surgery or liver transplant

  • Cause kidney failure (Hepatorenal Syndrome)

  • Worsen fluid overload (Ascites, Pleural Effusion)

  • Affect survival outcomes

How is it Diagnosed?

At Liver ICU or Hepatology Clinics, we do:

  • Echocardiography (Heart Sonography)

  • ECG

  • NT-proBNP Blood Test (Heart Stress Marker)

  • Cardiac MRI (if needed)

Treatment & Management

“A healthy liver needs a healthy heart.”

Key Approaches:

  • Salt & Fluid Restriction

  • Diuretics (under strict medical supervision)

  • Beta Blockers (if indicated)

  • Treat Underlying Liver Disease Aggressively

  • Monitor Heart Function Regularly

In advanced cases, patients may need:

  • Liver Transplant Evaluation

  • Cardiac Optimisation before Surgery

Prevention Tips for Liver Patients

  • Regular Liver & Heart Check-up
