MELD Exceptions — Explained Simply

MELD Exceptions — Explained Simply & Humanised

By Dr. Chetan Kalal — First DM Hepatologist of Maharashtra

(Expert in Liver Transplant, Hepatology & Advanced Liver Care)

What is MELD Score?

MELD (Model for End-Stage Liver Disease) Score is a numerical tool used globally to prioritise patients for liver transplantation.

It is based on blood tests:

  • Bilirubin (liver function)

  • Creatinine (kidney function)

  • INR (blood clotting ability)

Higher MELD Score → Higher chance of death without a transplant → Higher priority for liver transplant.

Then Why MELD Exceptions?

"Not every liver patient fits into the numbers game. Some life-threatening conditions are not reflected in routine MELD blood tests."
Dr. Chetan Kalal

These patients deserve MELD Exceptions — where extra points are granted despite low laboratory MELD scores because their conditions are equally or more severe.

Common MELD Exception Scenarios in Liver Transplantation

Condition Why MELD Exception is Needed?
Hepatopulmonary Syndrome (HPS) Severe oxygen deprivation not shown in MELD lab values.
Portopulmonary Hypertension (PoPH) High lung pressures, right heart strain.
Cholangiocarcinoma (Early Stage) Aggressive bile duct cancer within transplant criteria.
Primary Sclerosing Cholangitis with Recurrent Cholangitis High infection risk despite low MELD.
Familial Amyloid Polyneuropathy Progressive nerve damage needing liver transplant.
Hepatic Encephalopathy Refractory Severe brain dysfunction not reflected in MELD labs.
Intractable Pruritus (Severe Itching) Crippling quality of life despite low MELD score.

Process of MELD Exception Allocation

  1. Case Prepared by Transplant Hepatologist

  2. Submitted to Liver Transplant Committee / Authority

  3. Reviewed based on:

    • Disease severity

    • Documentation & reports

    • Ethical justification

  4. Extra MELD Points Granted (eg. MELD Upgrade from 15 to 22 or higher)

Why MELD Exception Awareness is Crucial?
