Silent But Deadly: Paracetamol Overdose — A Common Cause of Liver Failure

Paracetamol (also called acetaminophen) is the most commonly used over-the-counter painkiller and fever medicine.

But did you know — Paracetamol overdose is one of the leading causes of Acute Liver Failure (ALF) worldwide, including in India?

It can happen accidentally or in suicide attempts — and can be fatal if not treated on time.


How Does Paracetamol Cause Liver Failure?

Normally, the liver breaks down paracetamol safely.

But in overdose (especially more than 10-15 tablets or >150 mg/kg in a short time) — toxic metabolites accumulate in the liver and cause:

  • Liver cell death (necrosis)

  • Oxidative stress

  • Clotting failure

  • Multi-organ dysfunction


Symptoms of Paracetamol-Induced ALF:

Early (within 24 hours):

  • Nausea, vomiting

  • Stomach pain

  • Loss of appetite

Late (after 48-72 hours):

  • Jaundice (yellow eyes/skin)

  • Confusion / drowsiness

  • Bleeding

  • Kidney failure

  • Brain swelling (encephalopathy)

  • Coma
