Plasma Exchange (PLEX) in Acute Liver Failure (ALF)

Plasma Exchange (PLEX) in Acute Liver Failure (ALF)

Expert in Liver Failure & Transplant Medicine | Gleneagles Hospital, Mumbai

What is PLEX (Plasma Exchange) in Acute Liver Failure?

Acute Liver Failure (ALF) is a sudden and catastrophic condition where the liver rapidly loses its ability to function — leading to toxin accumulation, brain swelling, coagulopathy (bleeding), and multi-organ failure.

In this critical scenario, Therapeutic Plasma Exchange (PLEX) or Plasmapheresis emerges as an advanced liver support strategy.

Dr. Chetan Kalal, Maharashtra’s pioneering DM Hepatologist, explains:

“PLEX acts like an artificial liver, clearing harmful toxins, inflammatory cytokines, ammonia, and clotting factors from the blood — giving the liver a fighting chance to recover or bridging the patient to liver transplant.”

Why is PLEX Important in ALF?

Mechanism of Action:

In ALF, the patient’s plasma becomes toxic — filled with:

  • Ammonia

  • Inflammatory Cytokines

  • Immune Complexes

  • Bilirubin Overload

  • Coagulation Disturbances

PLEX removes this toxic plasma and replaces it with healthy donor plasma or albumin — supporting:

  • Brain protection (reducing cerebral edema)

  • Stabilizing clotting factors

  • Immune modulation

  • Metabolic correction

Dr. Chetan Kalal — Protocol for PLEX in ALF

Typical PLEX Protocol:

  • Sessions: 3-5 (Daily or Alternate Days)

  • Volume: 1.5-2 times patient’s plasma volume

  • Replacement: Fresh Frozen Plasma (FFP) / Albumin

  • Combined with: CRRT / Liver-specific medical therapy

PLEX is often used in combination with CRRT (dialysis), MARS (Molecular Adsorbent Recirculating System), or other liver support systems.

Benefits of PLEX in ALF Under Expert Care:

Goals of Therapy:

  • Stabilise the patient until liver regenerates (in drug or viral-induced ALF)

  • Optimise patient condition for emergency liver transplant

  • Reduce mortality in highly critical patients
