Who Needs a Liver Transplant?

Liver Transplant is advised when:
✔️ Liver is permanently damaged
✔️ No further medical treatment can reverse it
✔️ Quality of life is poor
✔️ Life expectancy is less without transplant


Common Indications for Liver Transplant

1. Decompensated Liver Cirrhosis

Cirrhosis with complications like:

  • Repeated fluid accumulation (ascites)

  • Variceal bleeding (vomiting blood)

  • Hepatic Encephalopathy (confusion)

  • Recurrent infections (SBP)

  • Poor nutrition & muscle loss


2. Acute Liver Failure (ALF)

Sudden liver failure due to:

  • Viral Hepatitis

  • Drug Overdose (Paracetamol, Rattol Poisoning)

  • Mushroom Poisoning

  • Herbal or Ayurvedic Drug-Induced Liver Injury

Time is life-saving in ALF.


3. Hepatocellular Carcinoma (Liver Cancer)

Early stage liver cancer within transplant criteria (Milan /UCSF Criteria) — offers best survival.


4. Metabolic or Genetic Liver Diseases

  • Wilson’s Disease

  • Primary Biliary Cholangitis

  • Primary Sclerosing Cholangitis

  • Budd Chiari Syndrome


How Do We Decide When to Transplant?

The decision is based on:

MELD Score

(Model for End-Stage Liver Disease)
Score based on blood tests.

MELD > 15 = Refer Early for Evaluation
MELD > 20 = Strong indication for transplant


Child Pugh Score

Another score based on:

  • Bilirubin

  • Albumin

  • INR

  • Ascites

  • Encephalopathy

Child Class C — usually needs transplant.


Urgency in ALF or ACLF

In Acute Liver Failure or Acute on Chronic Liver Failure — transplant is life-saving within days.
