What is MELD, and why does my score matter?
One of the first numbers patients hear once transplant enters the conversation is MELD — Model for End-Stage Liver Disease. It's a lab-based measure estimating how severe advanced liver disease is, and in deceased-donor transplantation, it helps assess clinical urgency.
Depending on the version used, MELD incorporates:
- Bilirubin — how well the liver processes and excretes bile
- INR — how well the liver produces clotting factors
- Creatinine — kidney function, closely tied to prognosis in advanced liver disease
- Sodium — significant hyponatremia is linked to worse outcomes
- Newer versions, such as MELD 3.0, also factor in albumin and sex
The point isn't memorizing the formula — it's understanding that MELD is a tool for estimating severity. It is not a complete description of the patient.
Is there a MELD number at which I "need" a transplant?
I don't want patients thinking of this as "MELD 14 = no transplant, MELD 15 = transplant." Medicine isn't that binary.
For routine deceased-donor listing in India, the Liver Transplantation Society of India's national consensus generally references a laboratory MELD around 15 for adults with decompensated cirrhosis, absent another transplant indication. That number is not a universal switch. A patient with a lower MELD may still have a serious transplant indication the score doesn't fully capture. A high MELD doesn't mean transplant automatically proceeds without weighing overall condition, comorbidities, infection, malignancy, frailty, and psychosocial readiness.
The score informs the decision. It doesn't make the decision by itself.
When the number doesn't tell the whole story
Two patients can carry similar MELD scores and be clinically very different. MELD doesn't fully capture:
- Severe or recurrent hepatic encephalopathy
- Refractory ascites
- Hepatic hydrothorax
- Significant muscle wasting and sarcopenia
- Recurrent infections
- Frailty
- Certain complications of portal hypertension
This is recognized in Indian transplant policy itself — the national allocation consensus incorporates cirrhosis complications, HCC, acute organ failure, and waiting time alongside MELD, in what's described as a composite clinical severity approach, with additional weight given to complications like frequent large-volume paracentesis, hepatic hydrothorax, and recurrent encephalopathy.
A MELD score is a number. The patient in front of me is not.
MELD and deceased-donor allocation in Maharashtra
This distinction matters especially for patients in Maharashtra: being listed for transplant and actually receiving a deceased-donor liver are not the same thing.
In Mumbai, deceased-donor liver allocation is coordinated through the Zonal Transplant Coordination Centre (ZTCC). Under ZTCC Mumbai's framework, a liver is first considered for suitable recipients on the local priority list of the hospital where it was retrieved. If no suitable recipient exists there, it moves to the city waiting list, and from there to the broader Maharashtra state-level pathway when appropriate.
What I actually tell patients
"Don't chase a number. Understand your disease."
If your MELD is rising, we need to understand why. If your MELD is relatively low but you have repeated hospital admissions, refractory ascites, hydrothorax, recurrent encephalopathy, or another major complication, I don't dismiss those problems just because the number looks reassuring.
If you're being considered for deceased-donor transplantation, your clinical urgency needs to be understood within the actual allocation framework governing your region — not a calculator found online.
A transplant decision rests on trajectory, complications, overall fitness, expected transplant benefit, and allocation rules — not one laboratory score in isolation.
