When it comes to natural remedies, apple cider vinegar (ACV) is one of the most talked-about — especially in relation to liver health. But does it actually help? Or is it just another wellness trend?
As a liver specialist, I’m often asked by patients whether apple cider vinegar can help with fatty liver, detoxification, or overall liver function. Here's what current science tells us — based on real studies, not myths.
𧬠What the Research Says
β May Help Reduce Fat in the Liver — in Animal Studies
Several animal studies have shown that ACV can:
Lower cholesterol and triglyceride levels
Reduce fat accumulation in liver cells
Improve antioxidant activity in the liver
These results are promising, particularly in models of non-alcoholic fatty liver disease (NAFLD). However, these studies were done in rats and mice, not humans — so while the findings are encouraging, we must be cautious in drawing direct conclusions.

Clinical studies in people (especially those with type 2 diabetes) have shown that daily use of diluted ACV may:
Lower fasting blood sugar and HbA1c
Improve LDL cholesterol and total cholesterol levels
These metabolic improvements can indirectly benefit liver health, particularly in patients with insulin resistance, obesity, or early-stage fatty liver.
π« No Evidence That ACV "Cleanses" or "Cures" the Liver
Despite what you may read online, there is no clinical evidence that ACV:
Detoxifies the liver
Reverses cirrhosis
Cures hepatitis or advanced liver disease
Liver "cleanses" or "flushes" are not medically supported. Your liver naturally detoxifies your body — and doesn't need external help to do so.
β οΈ How to Use ACV Safely (If You Choose To)
If you're considering using ACV as part of your health routine, here’s what I advise:
Limit intake to 1 tablespoon (15 ml) per day, diluted in a full glass of water
Avoid drinking it undiluted, as it can damage tooth enamel and irritate your throat or stomach
Take it with meals to avoid acid reflux
If you have ulcers, kidney disease, or are on medications (especially for diabetes or heart conditions), consult your doctor first

Clinically proven treatments
Balanced diet and lifestyle
Weight management
Alcohol moderation
And when needed, timely medical intervention
Apple cider vinegar may offer mild metabolic support, particularly in early stages of fatty liver or for patients with insulin resistance. However, it should never replace proper medical care or prescribed treatment.

It may help improve blood sugar and cholesterol in humans — which indirectly supports liver health
It does not cure liver disease or “detox” the liver
If used, it should be in moderation and under guidance, especially if you have other health issues
