🔍 What Are Autoimmune Overlap Syndromes?
Autoimmune liver diseases occur when the body’s immune system mistakenly attacks its own liver cells or bile ducts. In some patients, features of two or more autoimmune liver conditions are present simultaneously—a situation called an overlap syndrome.
The most common overlaps include:
Autoimmune Hepatitis (AIH) + Primary Biliary Cholangitis (PBC)
AIH + Primary Sclerosing Cholangitis (PSC)
Less commonly, all three may coexist

Because overlap syndromes don't fit into a single box, diagnosis requires:
Detailed blood tests (autoantibodies, liver enzymes, immunoglobulin levels)
Liver biopsy – to determine the dominant disease pattern
Imaging: Ultrasound, FibroScan, or MRCP if PSC is suspected
Colonoscopy – to screen for IBD in PSC overlaps

Treatment depends on the dominant component of the overlap:
🔹 If AIH is dominant:
Steroids (Prednisolone) and Azathioprine
🔹 If PBC is present:
Add Ursodeoxycholic acid (UDCA)
🔹 If PSC overlap exists:
Focus on bile duct monitoring, endoscopic care, and transplant planning
