What are Perioperative Complications in Liver Transplant?

These are medical or surgical problems that may arise:

  • Intra-operatively (during surgery)

  • Immediate Post-operatively (within 7-10 days)

Timely recognition & expert ICU management is the key.


1. Bleeding & Coagulopathy

→ Liver patients already have deranged clotting profile.
→ Surgery may involve significant blood loss.

Risk Factors:

  • Portal Hypertension

  • Coagulopathy

  • Major Vascular Dissection

Management:

  • Blood Product Support (PRBC, FFP, Platelets)

  • Thromboelastography (TEG) Guided Correction

  • Prevent Hypothermia & Acidosis


2. Primary Graft Non-Function (PGNF)

Rare but life-threatening.

Causes:

  • Poor Donor Liver Quality

  • Ischemia-Reperfusion Injury

  • Severe Hypotension

Management:

  • Early Diagnosis

  • Listing for Emergency Re-Transplant


3. Vascular Complications

Most dreaded post-transplant complications:

  • Hepatic Artery Thrombosis (HAT)

  • Portal Vein Thrombosis (PVT)

  • Hepatic Vein Outflow Obstruction

Symptoms:

  • Sudden Liver Dysfunction

  • Rising Liver Enzymes

Management:

  • Doppler Monitoring

  • Emergency Interventions (Thrombectomy / Angioplasty)


4. Biliary Complications

The most common long-term complication but can present early.

Includes:

  • Bile Leak

  • Biliary Strictures

  • Bile Duct Necrosis

Management:

  • ERCP / Stenting

  • Drainage Procedures

  • Re-surgery if Required


5. Renal Dysfunction

Causes:

  • Pre-existing Kidney Injury (HRS)

  • Intra-op Hypotension

  • Calcineurin Inhibitor Toxicity

Management:

  • Optimised Fluid Balance

  • Renal Replacement Therapy (CRRT / Dialysis)

  • Drug Dose Adjustments


6. Infections

Liver transplant recipients are highly immunosuppressed.

Early Risk:

  • Bacterial Infections

  • Fungal Sepsis

  • Catheter Related Infections

Late Risk:

  • CMV / Viral Reactivation

Management:

  • Antibiotic Stewardship

  • Strict ICU Infection Control

  • Targeted Prophylaxis


7. Pulmonary Complications

→ Ventilator Associated Pneumonia
→ Pulmonary Edema
→ Pleural Effusion

Management:

  • Protective Ventilation Strategy

  • Early Weaning from Ventilation

  • Chest Drainage


8. Cardiac Events

→ Cirrhotic Cardiomyopathy may worsen during surgery.

Complications:

  • Arrhythmias

  • Stress Cardiomyopathy

  • Heart Failure

Management:

  • Pre-op Cardiac Evaluation

  • Intra-op Monitoring

  • Early Cardiac Support
