Portopulmonary Hypertension (PoPH) in Chronic Liver Disease (CLD)

Portopulmonary Hypertension (PoPH) in Chronic Liver Disease (CLD)

Expert Insights by Dr. Chetan Kalal — First DM Hepatologist of Maharashtra

What is Portopulmonary Hypertension (PoPH)?

Portopulmonary Hypertension (PoPH) is a rare but serious complication seen in patients with chronic liver disease (CLD) and portal hypertension.

In simple words — PoPH means high blood pressure in the lungs’ blood vessels caused due to long-standing liver disease.

It affects the heart-lung-liver connection, making liver transplant and patient outcomes more challenging.

Why is PoPH Important?

Dr. Chetan Kalal — a leading liver specialist and the first DM Hepatologist of Maharashtra — emphasizes that PoPH is often underdiagnosed in cirrhosis patients.

“Breathlessness in a liver patient should not always be blamed on ascites or anemia — PoPH should be ruled out early for better survival and liver transplant outcomes.” — Dr. Chetan Kalal

How Does PoPH Develop?

  1. Liver Cirrhosis → Portal Hypertension

  2. Hormonal & Vascular Changes → Pulmonary Artery Constriction

  3. Increased Pulmonary Artery Pressure → Right Heart Strain

Symptoms of PoPH in Liver Patients

  • Unexplained Breathlessness (Dyspnea)

  • Fatigue on exertion

  • Chest discomfort

  • Dizziness or Fainting

  • Signs of Right Heart Failure (Leg swelling, Raised JVP)

How is PoPH Diagnosed?

Screening:
  • Transthoracic Echocardiography (2D Echo) — Look for raised pulmonary pressures (>40 mmHg)

Confirmation:
  • Right Heart Catheterization — Gold Standard

  • Pulmonary Artery Pressure > 25 mmHg confirms PoPH

  • Exclude other causes like COPD, Asthma, Cardiac disease

Severity of PoPH

Severity Pulmonary Artery Pressure (mmHg)
Mild 25-35
Moderate 35-45
Severe >45

Treatment Approach

1. Medical Therapy (Before Transplant)
  • Pulmonary Vasodilators

    • Sildenafil

    • Bosentan / Ambrisentan

    • Prostacyclin analogues

Goal → Reduce Pulmonary Pressure <35 mmHg before Liver Transplant

2. Liver Transplantation
  • Carefully Selected Cases after Optimization

  • Better Outcomes if PoPH controlled pre-transplant

Prognosis

  • Untreated Severe PoPH → High Risk of Mortality

  • Treated PoPH with optimized medical therapy → Liver Transplant offers good long-term survival

Dr. Chetan Kalal’s Expert Message for Liver Patients

“PoPH is not a death sentence — Early diagnosis, right treatment, and liver transplant can offer a new life to these patients. Breathlessness in CLD must never be ignored.”
