🧬 What is Primary Biliary Cholangitis?

Primary Biliary Cholangitis (PBC) is a chronic autoimmune liver disease in which the body’s own immune system slowly destroys the small bile ducts inside the liver. Over time, this leads to bile build-up (cholestasis), inflammation, fibrosis, and eventually cirrhosis.

PBC was formerly known as Primary Biliary Cirrhosis, but the name was changed because not everyone with PBC develops cirrhosis, especially with early diagnosis and treatment.


👩‍⚕️ Who Is Most Affected?

  • Women account for nearly 90% of cases

  • Usually diagnosed between the ages of 35–60

  • Can occur in people with other autoimmune disorders (thyroid disease, Sjögren’s syndrome, lupus)


🔍 Early Signs & Symptoms

Many people have no symptoms early on, but over time, they may experience:

  • Fatigue (extreme and persistent)

  • Itching (pruritus) — often worse at night

  • Dry eyes and mouth

  • Upper right abdominal discomfort

  • Dark urine & pale stool

  • Jaundice (in later stages)

The fatigue and itching are often misattributed to stress or allergies, delaying diagnosis.


🧪 Diagnosis: How Is PBC Detected?

Diagnosis is usually made through:

  • Blood tests:

    • Liver enzymes (ALP – alkaline phosphatase)

    • Anti-Mitochondrial Antibodies (AMA) – positive in 90–95% of patients

  • Liver function tests

  • FibroScan or MR Elastography to assess liver stiffness

  • Liver biopsy (in uncertain cases)


🩺 Treatment Options

While PBC has no cure, early and effective treatment can slow disease progression and improve quality of life:

🧴 First-Line Treatment:

  • Ursodeoxycholic Acid (UDCA) – helps bile flow and slows damage

💊 Symptom Relief:

  • Cholestyramine for itching

  • Fat-soluble vitamin supplements (A, D, E, K)

  • Bone health management (as PBC patients are at risk of osteoporosis)


🥗 Diet & Lifestyle: Food as Medicine

Nutrition plays a supportive role in managing PBC:

✅ Foods to Focus On:

  • Antioxidant-rich foods: berries, green leafy vegetables, turmeric

  • Lean proteins: fish, eggs, dals

  • Bone-supportive: calcium-rich foods, vitamin D

  • Hydration: good bile flow needs proper hydration

❌ Foods to Limit:

  • High-fat fried foods (stress liver detox pathways)

  • Alcohol

  • Raw shellfish (risk of infections in cirrhosis)

  • High-sugar or ultra-processed foods
